Importing cactus to USA

I have had a package destroyed by us customs that was coming from Australia. I don’t want to give up on importing but just not sure how to do it properly. If anyone has some experience with this and is willing to explain the process I’d be very grateful! I’m attaching a pic of the form I think I need but can someone confirm or lmk what else I’m lacking?
